Ryan White Title 1 Case Management
HIV+ individuals who meet the eligibility criteria will be provided with
comprehensive case management services. In a timely and coordinated process
we link the client with medically appropriate levels of health care, psychosocial
and other services. We work with the client and other family members in
focusing on continuity of care and an on-going advocacy assessment. For more

Ryan White Title I/Minority AIDS Initiative Client Advocacy Program
(CAP)
ITT Client Advocacy Program provide support assistance to HIV/AIDS
infected and affected individuals and their family which including legal, primary
medical care, medication, bereavement and caregiver supportive services,
HIV/AIDS education and information, and other needed services as identified
by client/case manger. Client Advocacy does not involve coordination and

Medicaid Application Enrollment Center
The Application Center provides outreach to individuals and families by
interviewing and completing an initial application for Medicaid. For more

HOPWA
Housing Opportunities for Person with AIDS, HOPWA funding provides
housing assistance and related supportive services as part of HUD's
Consolidated Planning initiative that works in partnership with communities and
neighborhoods to address the specific needs of persons living with HIV/AIDS
